About us

Community Services by Corumbene Care is made up of two main areas, firstly the Rural Primary Health Program which delivers services that improve the health outcomes of people with chronic conditions in the following local government areas;

  • Derwent Valley
  • Central Highlands
  • Southern Midlands

You are eligible to be part of our program if you have one of the following conditions;

  • Circulatory and heart disease (Cardiovascular Disease)
  • Chronic lung disease (including COPD and asthma)
  • Musculoskeletal disorders
  • Diabetes

RPHP can help you better understand and manage your health conditions, set realistic goals to improve your well being and access the services you need to support you at home. We also offer the following services;

  • Allied Health
  • Care Coordination
  • Telemonitoring

There is no age restrictions or cost to be part of RPHP. Referrals can be made by your GP or you can self refer.

Our other area in Community Services is Help at Home, which provides assistance to help you retain the independence of living in your own home.

All our services are flexible. If your requirements change, we’ll adjust your services to suit your needs.

Help at Home offer the following Government Funded Home Care Packages;

Level 1-2: Low Care & Support Needs
Level 3: Medium Care & Support Needs
Level 4: High Care & Support Needs

Help at Home can provide services to Southern Part of Tasmania and offer the following options tailored to your needs;

  • Assistance with meals
  • Transport assistance
  • Personal and private care
  • Support to maintain an active social life
  • In-home and residential respite
  • Home maintenance and gardening
  • Delivery of funded and private services
  • Carer support

We also offer Commonwealth Home Support Packages:

Short term assistance to help elderly Australians to remain at home:

  • Transport
  • Allied Health assessments
  • Flexible Respite
